Our clinic is a dynamic place to work, practice, and grow. We have 4 primary care health centers and deliver integrated services including Medical, Behavioral Health, Pharmacy, Dental, specialty referrals, chronic disease management, health education, and much, much more.

  • Wage depends on experience
  1. Must have RN (Registered Nurse) licensure/ or Bachelors degree in Public Health
  2. Requires 3 years of experience in your related field. Requires knowledge of the care management assessment technique, provider resources, eligibility requirements, and community resources.
  3. Must have strong oral, written, and interpersonal communication skills, PD skills to include word processing, spreadsheet, and database applications, organizational and problem-solving skills. This position deals with complete cases and must have knowledge of chronic care terminology.
Requirements
  • Ability to communicate in Navajo is a preferred, but not required. Will be working in a rural outpatient clinic in Monument Valley, Utah
  • Must be able to work well with others
  • Must have an excellent work history and customer service mindset.
